The telehealth assessment will provide key data to help enhance the tribe’s access to health care across the region

The Sault Ste. Marie Tribe of Chippewa Indians is asking tribal members to take part in a telehealth survey that will be used to assess and improve local access to health care. The surveys should be arriving throughout this week.

These surveys provide valuable information that helps with programming and acquiring grant funding.

The telehealth surveys are being done in partnership with Connected Nation Michigan (CN Michigan) and the Michigan Health Endowment Fund. The 15-minute surveys can be found online by heading to this link: https://surveys.connectednation.org/s3/Sault-Telehealth.

Your personal data is protected. CN Michigan does not use or share any data collected through this survey for any marketing purposes. The nonprofit will not sell any information collected through these surveys. No personally identifiable information, individual household survey responses, or information that could be used to identify a household and its survey responses are shared with any entity outside of this project. No names, addresses, or respondent demographic information are shared with any outside entity without the express consent of the individual.
