FREQUENTLY ASKED QUESTIONS—COVID-19 Vaccination Lottery.

Q: HOW DO I KNOW MY APPLICATION HAS BEEN RECEIVED?
A: You should have received a confirmation email upon your online application submission. Be sure to check your spam folder as well. If you did not receive a confirmation email, then you should resubmit your online application.

Q: I AM A TRIBAL MEMBER AND A TEAM MEMBER, WHY DO I ONLY FILL OUT ONE APPLICATION?
A: When you chose this option at the top of your application (“am currently an employed Team Member AND I am currently an enrolled Member of the Sault Ste. Marie Tribe of Chippewa Indians, and I am eighteen (18) or older (Tribal Member Drawing AND Team Member Drawing”)), you will automatically be entered into the two separate drawings.

Q: DO I NEED TO APPLY AGAIN EACH MONTH?
A: No, your application will continue to carry over for each month, no need to resubmit.

Q: CAN YOU MAIL ME AN APPLICATION?
A: No, applications are only being accepted online as the drawing will be held by a third party.

Q: I JUST QUIT MY JOB LAST WEEK, CAN I STILL APPLY?
A: No, only current team members can win, verification will be required. Employees will need to show proof they were employed at the time the Entry Form was submitted AND as of the drawing date.

Q: WHAT IF I QUIT MY JOB AFTER I APPLIED?
A: You will no longer be eligible to win. Employees will need to show proof they were employed at the time the Entry Form was submitted AND as of the drawing date. (If you are also an enrolled tribal member you will still be eligible to win in that drawing.)

Q: I MADE A SERIOUS ERROR ON MY ENTRY FORM. HOW CAN I CORRECT IT?
A: Call 800‑793‑0660 and ask for a membership liaison. Tell them you made a serious error on your form that needs to be corrected. The liaison will verify your identity and help you fill out a new form.

Q: I RECEIVED A BOOSTER, CAN I APPLY MORE THAN ONCE?
A: No, each person can only apply once. Tribal members who are also team members may also only apply once, but they will be automatically enrolled in both drawings if they select the corresponding box in the Entry Form. More than one entry in either drawing will be removed from the drawing.

Q: I LIVE OUT OF STATE, DO I QUALIFY?
A: Yes, all tribal members that meet the vaccination requirements are eligible to apply.

Q: I DO NOT HAVE AN EMAIL ADDRESS. CAN I STILL SUBMIT AN ONLINE COPY?
A: Yes, you can still apply regardless of whether or not you have an email address. In this case, be sure to check for the accepted status on the page after you submit to confirm your application was received. The names of the drawing winners will also be posted online.

Q: What can I do to correct an error on my Lottery Entry?
A: Submit another Lottery Entry but click on the button next to "A Corrected Lottery Entry." This is just above the signature area. Also, add any information such as the Reference # that was in the email confirmation of a previous entry if you have it and any information about the incorrect entry you would like removed.
