NEW REQUIREMENT!

Substitute Form W-9 (Taxpayer Identification Number Request)

If you are a NEW STUDENT to our programs, or have a NAME OR ADDRESS CHANGE you are required to fill out a W-9 form.

We are required by law to obtain this information in order for our Accounting Department to have correct and updated information on every student we pay in our Self-Sufficiency Incentive program. This is in conjunction with the Income Tax Form 1099 (see below).

Sault Tribe Accounting Department WILL NOT process your Self-Sufficiency Incentive Check without this form!

• Print off the W-9 form
• Fill out the INDIVIDUAL section (Individual’s Legal Name and Social Security Number) and ADDRESS information (at the bottom)
• Make sure your signature is on the form
• Fax or mail it to our office – the contact information is on the form!

Income Tax Form 1099 (Miscellaneous Income)

The Sault Tribe's Accounting Department will be sending students who receive incentive award(s) a 1099 Form that shows how much the student received during a calendar year (January - December). These will be sent in late January.

For example, if a student gets a check in January 2006 and another one in May 2006, the total amount will be on the 1099 Form and will need to be reported on the following year's income taxes. This is considered income because the check is given directly to the student.
